Youth Stipend and Mileage Reimbursement Forms

 

Instructions:

 

• Stipend requests for youth co-presenters, panelists, guest presenters, and participants must be made at least 2 weeks prior to the training, to ensure enough time for stipends to be delivered.

• All stipends must be signed for by the youth when they receive them. Trainers must use the stipend receipt form to collect these signatures.

• Mileage reimbursement is available at the rate of $0.54/mile for all youth or supportive adults/peers who transport youth from out of town. To request mileage reimbursement, drivers must fill out the form at the event. Trainers must then submit these requests after the training.
